Human Services and Public Health Department - HENNEPIN COUNTY provides Rapid Re-Housing moves people directly from homeless cases to their own rental unit, providing short-term financial assistance in the form of initial rentals, security deposits, and/or utility assistance. Columbus House has been a Housing First agency since 2005, and in 2009 began offering a rapid re-housing service. Supporting Housing - Congregational Web Sites: Columbus House serves nearly 250 people in private owner's apartments, for which the client receives a proof of state or federal housing. Scattered housing programs on the site encourage clients to reintegrate into the wider community, while giving them the personal space to promote greater independence. Connecticut Collaborative for Reentry (CCR) - Formerly known as Frequent User System Engagement (FUSE), CCR serves people who have recently been released from prison and are at risk of homelessness, all as part of the state's "Second Chance Society" initiative. Social Innovation Fund (SIF) - As part of a federally funded program, Columbus House works with partners across the state to provide case management services and supportive housing for clients who already have access to health care through a federal program.
