Five Horizons Health Services (FHHS) offers HIV prevention, testing and advocacy, Specialized Medical Treatment Options such as HIV Pre Exposure Prophylaxis, “PrEP”, Hepatitis C, Sexually Transmitted Disease and Infection Diagnosis/Treatment and general medical care. The services include, but are not limited to, housing assistance(rent/mortgage), medication payment assistance, transportation assistance, procurement of food and personal hygiene items, support groups, and referrals as necessary. Social workers address immediate needs, as well as those concerning long-term self-sufficiency.
